What is the amount of my lay employee life insurance coverage?

A term policy in the amount of $10,000, which reduces to $6,000 at age 70 for eligible lay employees.

How do I add or change a beneficiary on my life insurance policy?

Retirees: Complete a new designation of beneficiary form found on the link under Resources and Forms and return the form to the Benefits Office.

Active Employees: Log in to ADP and go to “Myself” at the top bar – “Benefits” – “Enrollments” – “View Benefits” and select the 3 dots to the right of your Life Insurance enrollment to edit.

Can I enroll in only the life insurance even if I don’t enroll in the medical insurance?

Yes, you may enroll in the life insurance as a new hire, part-time to full-time status change, or during the annual open enrollment. The monthly premium rate is $2.00.

Can I purchase additional coverage for myself or for a family member?

Yes, you may purchase additional coverage through our Voluntary Supplemental Life Plan with Guardian. Child and Spouse plans are also available, please see the Voluntary Supplemental Life section for more details.

When I retire will I still have life insurance?

If you are eligible for retiree medical benefits and continue enrollment in the medical plan, you are eligible for a term policy in the amount of $10,000, which reduces to $6,000 at age 70. If you are not eligible for retiree benefits, your life insurance will terminate as of your termination date.

Upon my termination, can I convert my life insurance benefit?

Yes, you have 31 days from the date of your termination date to contact Dearborn Group at (800) 348-4512 regarding conversion to an individual policy.

A voluntary term policy available for purchase in increments of $10,000 from $10,000 to $200,000 for eligible lay employees.

Yes, coverage for a Spouse and Children under 26 is available. For a Spouse, you may purchase $5,000 to $25,000 in $5,000 increments as long as you have two times the amount of coverage on yourself through the Guardian Voluntary Life plan. For Children, you may purchase $2,000 to $10,000 in increments of $2,000, as long as you have two times the amount of coverage on yourself through the Guardian Voluntary Life plan. Please note that any enrollment or increase outside of your first new hire/new full-time status enrollment is subject to Evidence of Insurability.

When I retire will I still have life insurance?

No, this coverage will terminate upon termination of employment.

Upon my termination, can I convert my life insurance benefit?

You may reach out to Guardian at 1-888-482-7342 to convert your life insurance benefit upon termination.
